Summary The purpose of the Level of Care (LOC) assessment is to establish the level of care a developmentally disabled person may need. This may be needed for admission to an intermediate care facility or enrollment in home or community based services (HCBS) waiver. The LOC assessment is not intended to be a comprehensive assessment; rather, it gathers relevant information related to functional eligibility. It serves as a foundation for comprehensive assessment and service planning. Other parts of the LOC tool provide data related to federal and state outcomes.
